Penile Lengthening 101

Penile length has been a source of male pride and pleasure since the beginning of human history. While your penis length is the appropriate response to the Pop culture query for “How big are you?”, it’s increased girth that’s been proven to drive the most orgasmic response from a vagina during intercourse.

The simplest way to increase your penis length is through improved erection performance. The more engorged your penis is – the longer it becomes. It’s this fact that makes “penis enlargement pills” legal to sell. All they have to do is include a known supplement that supports improved erections and by definition: it’s a “penis enlargement pill”.

Unfortunately, beyond improved erections there’s no effective way to increase the length of a penis without surgery beyond sub-millimeter improvements. Every procedure to increase penile length will include a release of the ligaments that attach to the base of the penis organ from the pelvic bone.

Important Details Regarding Release of Ligaments at Base of Penis for Length

• Any form of surgical lengthening procedure that involves the release of ligaments release that does not include cutting both the top and deep suspensory ligament will almost never result in any real penile length increase.
• The primary motivation behind performing a ligament release procedure to increase penile length is to remove the structural supports that make it practically impossible to stretch the penis organ with a penile traction device. The penis organ has the consistency of a thick sponge. It’s not easily stretched – but it is stretchable. However, it’s nearly impossible to successfully stretch the organ with the “structural guy wires (The ligaments) still attached.
• If a penis doctor only removes the top ligaments and not the deep ligament, the outcome is akin to only cutting the top roots at the base of a tree you are trying to pull from the ground but ignoring the underlying tap root. It’s worth noting that in 2019 alone, Morganstern performed over ten different “repair” surgeries where patients actually “lost” length because their surgeon only released the top ligaments. Many of these patients were told the “ligament” reattached but our retrospective surgical analysis most failed because of only cutting the top ligament.
• Surgical ligament cutting performed through the scrotum (so as not to leave a scar above the penis) are doomed to fail because the surgeon can only access the top ligaments through this vantage point. Please see previous discussion regarding shortfalls associated with this approach.

If you were standing up straight and had a full penile engorgement, the erect angle would drop downward between 5-20 degrees, depending upon your present angle pointing upward when erect. In most cases men are situated on bedding when erect which makes this change irrelevant.
• MegaMAXL patients cannot get a permanent girth procedure at the same time as this procedure. We tried that combination on multiple occasions – the extensive lengthening procedure produces too much edema, which subsequently triggers failure for the fat tissue transplant for thickness. Permanent girth would need to be added via a subsequent procedure either before or after this maximum lengthening procedure

MegaMAXL Recovery & Aftercare Highlights

• The MegaMAXL requires that the surgeon work in an area up to two inches deep within the midsection for penile repositioning. Most men miss four days of work following this procedure and can’t lift anything over ten pounds for about three weeks post-surgery.
• The MegaMAXL results in a scar of approximately two inches running straight upward from the base of the penis. Uniquely, the resulting scar is almost identical in location and length to the residual scar left behind following a hernia operation. The scar eventually heals to a hairline width and can usually be covered by pubic hair. Morganstern MegaCONTOUR Penile Lengthening:

• The MegaCONTOUR provides a pathway for men with larger mid sections, pubic fat pads and/or patients with a mild or moderate buried or hidden penis diagnosis to achieve maximum penile lengthening in a singular procedure. It combines three procedures into a singular surgical appointment, including a pair of surgical lengthening procedures plus complete surgical removal of the fat pad above the penis along with subsequent tightening of the skin within the fat pad region. • Morganstern’s MegaCONTOUR procedure is an affordable game-changer for patients suffering from lost penile length related to an engorged pubic fat pad. Additionally, some patients already diagnosed with moderate buried or hidden penis will be able to procure a MegaCONTOUR procedure and regain their sex life and improve their self-image. Similar versions of this procedure performed at Emory University Hospital cost over $65,000 and health insurance does not cover that expense.
• Heavier patients where excess fatty tissue has migrated into the scrotum along with men with more severe versions of buried and hidden penis will not be candidates for the MegaCONTOUR procedure. The surgery time and 24-hour nurse assisted recovery aspects of those cases are not suited for an out-patient procedure. We will be happy to refer such cases to Emory University hospital for evaluation.

MegaContour Penile Lengthening Aspect Procedure Details

• The MegaCONTOUR includes a complete release of the suspensory ligaments at the base of the penis, as described in the previous Morganstern MAXL procedure summation
• In the MegaCONTOUR, we also perform a surgical “penile repositioning procedure” along with our full ligament release where we go through the lower abdomen and reposition an inch of your penis that presently goes straight back under the pelvis outward (you can reach down and feel this part of the penile organ with your fingers)
  • One immediate inch of penile length is visible after surgery, derived from the penile repositioning process. The second inch is achieved through the six-month penile stretching protocol from having released and optimized the ligaments
• The MegaContour procedure results in a mild change in the degree of upward angle with an erect penis. What is the Pubic Fat Pad?

The suprapubic fat pad (aka mons pubis) is the hair bearing area of fatty tissue located on the pelvic bone just above your penis.

What Causes an Oversized Pubic Fat Pad?

Most commonly, a fat pad becomes engorged in concert with significant weight gain. However, some people, even very skinny people, are genetically predisposed to maintain an oversized mons pubis.

Impact of Large Suprapubic Fat Pad on a Man’s Penis

Since the mons pubis is located directly above where the penis organ comes out from inside the body just under the pelvic bone, any increase in its size is going to start to visibly decrease the visual length of the penis. The problems faced by men with oversized mons pubis include:

1. In patients that have oversized pubic fat pads, the pad can begin to “pull” the base of the penis into the fatty tissue, causing the flaccid penis size to become smaller and a full length erection dependent upon “pressing down” of the base of the penis with your fingers in order to reveal its true length.
2. Vaginal penetration depth is compromised by the increased mass at the base of the organ
3. One recent analysis concluded that for every thirty-fifty pounds of excess weight added, a man will lose one inch of visible penis.
4. In more severe cases, the pad can completely envelope the area and bury the penis beneath the skin surface (aka buried penis / hidden penis).
5. Standing to urinate can become challenging or eventually require catheterization

Simply stated, as you gain weight and the pubic fat pad increases in size, what’s changing is how much of your penis organ is getting situated on the inside of your body vs. the outside. If you face any of these circumstance, you’re not alone. A recent article in the Atlantic revealed that 1 in 3 men are too fat to see their own penis.

Morganstern Surgery to Correct an Oversized Mons Pubis

The best option for patients dealing with an oversized pubic fat is the simplest: lose weight.

For patients who’ve been unsuccessful in achieving weight loss goals, Morganstern Urology offers three different surgical variations for a reduction in the pubic fat pad, based upon the mass of the fatty tissue, how the penis is presently affected by the condition, and if the patient needs or wants increased penile length.

Liposuction of the mons pubis is not recommended for men for a couple of reasons. Patients who’ve carried an enlarged pad for any extended time almost always have stretched / loose skin that will sag after liposuction of the pubic area – causing the penis to appear just as small – or even smaller. Secondly, there are four thousand sensory nerve endings in the end of the penis – all of which are “wired” through the base of the organ where the pubic fat pad is located. Traditional liposuction is not precise enough to succeed without running the risk of damaging the basis for some of these essential nerve endings. The pubic fat pad area also houses lymph nodes which don’t bode well with liposuction. When lymph nodes are inadvertently liposuctioned, they cause many problems with recovery and healing and sometimes additional surgery.

Mini-Fat Pad Removal

Our Mini-Fat Pad removal is available for an additional charge during lengthening procedures. Often our urologists can’t perform this procedure with our penile length and width enlargement PERMMAXL because of the risk for excessive edema which can negatively affect the long term viability of transplanted tissue for penile girth enlargement

Surgical Fat Pad Removal

MegaCONTOUR Fat Pad Removal & Penile Lengthening Procedure

Morganstern’s MegaCONTOUR procedure was discussed in detail in a previous section. It basically combines a surgical pubic fat pad reduction with a MegaMAXL penile lengthening procedure. It’s indicated for patients where simply reducing the fat pad will have only minimal effect with enhancing penile protrusion or in patients suffering from an engorged pubic fat pad plus a less than desired natural penis length.

Recovery from Fat Pad Reduction

Regardless of which version of pad reduction a patient chooses, there is almost always a great deal of swelling afterward. This swelling can extend down into to the scrotum. Bruising is also not uncommon. Bruising usually resolves after two to three weeks. Swelling can take longer to resolve, usually by six weeks. Continued improvement in swelling can continue for up to three to four months after surgery.

Possible Complications from Monoplasty Fat Pad Reduction

As is the case with any surgery, complications are possible after a monoplasty: hematoma⁷, seroma, deep vein thrombosis⁸, pulmonary embolism⁹, delayed wound healing, wound separation, skin necrosis, and wound infection are some of the possible complications that can occur after surgery to contour the pubic area.

Cosmetic Treatments for the Scrotum

Morganstern offers a pair of minor procedures to reshape and rejuvenate enhance the visual appeal of the scrotum.

Minor Surgical Option

Foundoplasty

• Another relatively common occurrence is a build up or excess of scrotal tissue around the base of the penile organ. This glut of tissue visually minimizes the length of the penis, particularly when flaccid. Removing it creates a longer-looking and more aesthetically appealing penis. Removal of built up scrotal tissue at the base of the penis. Removed tissue reconstituted under base of penis to help improve visible length.
• The Morganstern Foundoplasty^TM is a minor surgical procedure that frees the penis from the retracted state from excess tissue at the base of the penis. A portion of the excised tissue is repurposed under the base of the penis organ to cause it to jut-out a bit: making it appear even longer when flaccid.
• The Morganstern Foundoplasty^TM requires mild anesthesia of an epidural and IV sedation. The procedure takes about thirty minutes and most every patient is able to return to work within 48 hours after the procedure with only minimal discomfort. It’s one of the simplest and most innovative ways to improve penile length available.

Nonsurgical Scrotal Enhancement Option: Scrotox

For men with unnaturally high positioned testicles or patients suffering from testicular shrinkage from testosterone therapy or similar treatments, a popular solution is the injection of BOTOX® (appropriately, this procedure is referred to as “Scrotox) into strategic points within the scrotal area. This procedure relaxes the taut tissue and muscles providing a more natural relaxed appearance where the testicles hang freely. This is also a popular option for men with naturally high or contracted scrotums, as the active ingredient in BOTOX relaxes muscles and causes them to “hang loose.” Depending on where in the scrotum BOTOX® is administered, this approach can lead to a smoother scrotum, lower-hanging testicles that appear larger. Additionally, BOTOX® injections directly into the testicles has been shown to reduce sweating, if needed.

Although BOTOX® injected into the scrotum is not FDA indicated, thousands of successful treatments have created a strong safety record. A recent clinical study regarding the effectiveness of the BOTOX® injections into the scrotum revealed positive results. A Scrotox procedure only requires about 15 minutes to administer and patients can return to work and resume physical activity the same day as the procedure. Morganstern utilizes a topical anesthetic and the fineness of the needles cause almost undetectable pain during a treatment. Results usually last about four months after the procedure.

How Does Morganstern Urology Perform a Scrotox Procedure?

A Scrotox treatment is a fairly simple flow:

1. The surgeon will apply numbing cream to your scrotal sack
2. The surgeon will query you regarding any specific objectives to your Scrotox treatment
3. The surgeon strategically injects the Botox into select areas, repeating application in key spots
4. The surgeon will clean and disinfect any bleeding

This entire process takes about 15 minutes

How Long is the Recovery from a Scrotox Procedure?

Recovery from a Scrotox procedure is quick and relatively painless.
Scrotox is an in-office procedure. You come into our office, have your procedure completed and can usually return home within minutes after your case is complete. Additionally, there’s no need to take off the whole day for recovery. You can immediately return to most activities but we suggest you refrain from lifting heavy objects or engaging in sexual activities for 24 hours.

What are the Side Effects of a Scrotox Treatment?

The side effects from a Scrotox procedure are minimal. Some documented side effects include:

Bruising
Swelling
Feeling of tautness in the scrotal area
Numbness
A reduced sperm count was documented in a study of rats

How long does a Scrotox treatment provide results?

Scrotox results typically last about three to four months.

Circumcisions

• Circumcisions are a common pre-cursor to many of our augmentation procedures. If your circumcision was poorly executed at birth, you might need to get a scrotal lift before any lengthening procedure since a further advancement of that tissue along the shaft from added lengthening will cause the condition to worsen.
• We require every patient be circumcised before we perform any type of girth enhancement.
• Why is that?
• Since all girth enhancements are achieved through adding some form of bulking agent to the subcutaneous tissue surrounding the penis organ, there’s a strong likelihood that the bulking agent will eventually migrate to the foreskin during the normal activity of sexual intercourse and masturbation. Such a shift is unsightly and inhibits the healthy retraction of the foreskin. We have performed countless surgical repairs of patients who received a girth enhancement elsewhere that migrated to the foreskin. We recognize its bad healthcare and insist a patient get a circumcision before augmenting penile girth.
• Morganstern Master Circumcision
• We offer a master circumcision as an out-patient procedure at our clinic. If you can plan to add girth to your penis, we leave a little bit of extra skin near the distal portion of the shaft to create an even larger subsequent sickening procedure.
• A circumcision can’t be performed the same time as a girth enhancement as the healing pressure on the tissue works in opposite directions. A circumcision must be completely at least ninety days before any type of girth augmentation.

Case Consultation

Your journey begins with a private case consultation with one of our male medical advisors over the telephone. Their role is to educate you regarding the various pro’s and con’s with different procedures.

They will walk you through a series of questions regarding your health, lifestyle habits, physiology and size objectives to determine which procedures would be most appropriate in your particular case and educate you on the pro’s and con’s associated with every option.

They will make you feel very comfortable talking about all these matters – it’s what they do all do and some are former patients.

Their role is to educate and if you plan to more forward, help narrow you in on the one or two procedures that seem ideal for your scenario. They will schedule you for a clinical evaluation if you’re ready and suited for the next step.

Clinical Evaluation

The clinical evaluation is usually performed over the phone between the prospective patient and a urological surgeon. If you live nearby or will be traveling to Atlanta soon, you can schedule an in-office evaluation.

The objectives of the clinical evaluation are three-fold:

1. Confirm Suitability: The doctor will evaluate and confirm – medically speaking – if you’re a good candidate for the procedure you requested. If you’re trying to decide between two, he’ll help guide you to the one that he feels will provide the best outcome in your circumstances.
2. Answer Questions: The doctor will be available to answer any specific questions you have about your procedure.
3. Evaluate Contra’s: The surgeon will be checking to see if any health or urologic issues exist that must be addressed prior to your target procedure or that would make another option more suited to your case.

The doctor’s nurse will be on this call recording medical notes for your file and will take over the call at the close.

Patient Reflection and Perspective

Some patients have already done lots of homework and are ready to “pull the trigger” immediately after getting confirmation of their suitability from a clinical evaluation. Others need time to reflect upon their options and consider if penis augmentation is suited for their life and lifestyle.

Augmentation Procedure Scheduled

A $1,000 non-refundable deposit is required to schedule and secure an exact surgery date. There’s often about a one-month lead time on surgery dates so you’ll want to act sooner rather than later if you have a particular time frame in mind.

Patient Pre-Certifications and Preparations Completed

Depending upon your exact augmentation procedure and your existing physiology, there will, often be certain lab tests and certifications you’ll need to get completed before your actual procedure. Most can be completed by your primary care physician or a micro-clinic. Our request for information is only indicated as “an elective procedure” – no one will be aware any of your tests are associated with augmentation.

Travel Arrangements Completed

Ninety-five percent of our enhancement patients come from other states or countries. Thus, most all are traveling to get their procedure. While all of our procedures are out-patient procedures, most require at least one overnight stay to ensure proper healing before undertaking travel. We can provide you with alternative hotel properties near our facilities.

You will need to have someone drive you to and from our clinic post-procedure if your augmentation includes anesthesia but you can arrange for an Uber or Lyft if you don’t plan to have a friend or family member accompany you to your enhancement.

Pre-Operative Visit

We will need to schedule a pre-operative visit with your surgeon before your enhancement so that he can confirm aspects of your procedure and confirm your success in that regard. In rare cases, a change will be made to your exact procedure because of physiological aspects uncovered during a first-hand evaluation that were not evident from prior evaluations

Enhancement Procedure Time

The actual surgical time required to complete your procedure varies with what you’re getting done. At a minimum, surgical procedures will take at least three hours from arrival to departure. Some enhancements require more then twice that time.

Augmentation Recovery

Following your procedure, our staff will monitor your recovery at our clinic until you’re ready to be discharges to your transportation to more completely recover at a nearby hotel. The morning immediately after any surgical procedure is always the most pain-filled as the anesthetic wears off. We prescribe pain relief medications for select procedures to assist patients just after surgery.

Post-Operative Care

Based upon the augmentation you’re getting, there will be certain prescribed aftercare that’s essential to you achieving targeted outcomes. Our staff and your surgeon will detail these care procedures before and after your enhancement.

Follow Up Appointments (If Needed)

Your surgeon will usually schedule a post-operative appointment based upon your availability and your particular case. Some men stay a few days post procedure in order to meet with the doctor before heading back home. Others just touch base via phone and travel back home at their earliest opportunity.

Follow up can be performed in office or over the phone with photos, depending upon your circumstances. If you have any questions or concerns during your recovery, we ask that you take photos that help illustrate the issue so the doctor can best comprehend how to help you.

What Do Penile Augmentation and Rejuvenation Procedures Cost?

Penile rejuvenation surgery is a cosmetic urologic procedure that results in added penile size in either length, width – or both.

The cost varies with the exact procedure(s) performed. Non-surgical girth enhancements start at $7,100, while surgical thickening costs $8,900. Our most popular PERMMAXL providing both length and width in a singular procedure costs $11,500. Our MegaMAXL maximum lengthening procedure is $9,800. The MegaCONTOUR costs $13,800.

Our urology clinic understands that cosmetic penis surgery is expensive and that many men do not have the cash readily available. Dr. Morganstern’s office has arranged for financing options, making the procedure more accessible. Men can now undergo the surgery for as little as $3,000 down with approved credit. If you don’t have a good credit rating you can explore getting a co-signer for a loan, if applicable.

Is the Cost of Penis Augmentation Covered by Health Insurance?

Unfortunately, no. Penis enhancement is never covered by health insurance coverage regardless of circumstance. Some policies will cover the cost of a circumcision by your local doctor but nothing within enlargement aspects.

Penis augmentation is considered an “elective” procedure like a facelift or cosmetic breast augmentation.
