How is this Low Testosterone Determined and Treated?

Low testosterone is easy identified via a simple blood test done in Morganstern Health’s Atlanta office in Buckhead. If the test determines that you have low testosterone, treatment is a minor surgical procedure and implantation with minimal discomfort. This discomfort is often treated with Ibuprofen (Advil) which typically does not interfere with your work schedule.

How often will I need pellets to improve my condition?
For women: usually every 3-4 months.
For men: usually every 4-5 months.

Why are pellets better than creams, patches, shots or pills?
Pellets are pure hormone that is not metabolized into by products by going through the liver, stomach or skin. This delivery system allows your body to use the right amount of hormone as the blood flow surrounding the pellets pick up what is needed.

Where do the pellets go?
Because they are pure hormone without fillers or synthetic ingredients, they completely dissolve.

How long will it take for my body to become leaner utilizing hormone pellets?

Why do I need estrogen?

Will I grow unwanted hair from testosterone?
There is less chance of excess hair growth with natural testosterone than with synthetic hormones. Facial hair will grow with testosterone pellets, but normally no more than when you were in your thirties.

Does testosterone cause prostate cancer?
No. Metabolites of testosterone, Dihydrotestosterone and estrone cause prostate enlargement and contribute to prostate cancer. Estrone increases and testosterone decreases as men age and gain belly fat. Testosterone pellets are the only replacement that reverses that trend.

How do I take testosterone pellets and not convert them to Dihydrotestosterone and estrone?
Your blood levels of estrone and DHT are checked after treatment to see if they are elevated. Some men still convert to these metabolites even on testosterone pellets. If they are converting, we troubleshoot with natural supplements such as DIM and Saw Palmetto, or an aromatase inhibitor prescription.

What if I have prostate enlargement already?
Testosterone pellets will make it better, if you do not convert it to DHT; we will treat that if it happens.

Will my testicles shrink while on testosterone pellets?
Yes, they will. Testicles are suppressed by taking any kind of testosterone and will not make as much testosterone while the pellets are working. This is not permanent and the testicle retains its ability to produce testosterone.

How are BioTE® Hormone Pellets Created?
Pellets compounded and used in the BioTE Method are made of pure Human Identical hormone and a small amount of Stearic Acid that acts as a binding agent to keep the hormone in pellet form for insertion.

If you’ve ever taken an Aspirin you’ve had stearic acid, and there are no known allergic reactions to stearic acid.

Hormones are “synthesized” in an FDA Certified lab. This “synthesis” or creation of “Human Identical Hormones” is very different than “Synthetic Hormones” which are different in molecular structure than the exact identical molecular structure of the Testosterone and Estradiol molecule that our bodies naturally produce.

Carbon, Hydrogen, and Oxygen atoms are taken from plant matter such as Soy or Yams (because we have to get the atoms from somewhere) to make the molecules known as Testosterone and Estradiol.

Once this human identical molecular structure is created, it no longer resembles anything from which it was derived, no matter what the source of the atoms were originally. Under the microscope, the Testosterone and the Estradiol that is in the pellet is the exact same molecular structure as the Testosterone and Estradiol that we have in our own physiology.

Pesticides, GMO and Antibiotics Concerns/Risks
There is no genetic material in atoms; therefore, they cannot be genetically modified so concerns around GMO, Pesticides, Antibiotics, Herbicides, etc. are of no concern because the atomic structure of an atom cannot be altered with exposure to those substances. Also, there is no concern about the originating source and allergies, since the resultant molecule is now either testosterone or estradiol and no longer resembles the originating molecule. Hydrogen will always be Hydrogen, Carbon will always be Carbon, and Oxygen will always be Oxygen.

Our pharmacists are able to make (synthesize) anything in the laboratory or compounding pharmacy to resemble anything that exists in nature and not in nature, in this case, Testosterone and Estradiol. The human identical hormone that is in the pellet is just that….Human Identical Hormone and does not resemble or have any of the characteristics of the source material (plant matter in this case) from which it was created.
In summary, the body knows exactly how to best utilize the pellet of “human identical hormone” with very few adverse reactions or side effects, because it is the same molecule that it has been naturally producing.

What does Hormone Replacement Therapy Treat in Men and Women?
Hormone replacement therapy (HRT) is used to treat menopausal symptoms in women and andropause symptoms in males.

Most women who take HRT for menopausal reasons are given an estrogen/progesterone/testosterone combination, except those who have had a hysterectomy, as they may not need progesterone. HRT has shown to reduce fatigue, improve sleep, improve libido in women and sexual performance in men, decrease muscle loss and reduce body fat (Staland 1978, Thom 1981, Brincat 1984, and Davis 1995). It also has been shown to reduce irritability, anxiety and depression. The symptoms of osteoarthritis and rheumatoid arthritis are significantly reduced. Long term, men and women will have reduced incidence of Alzheimer’s disease, heart disease, and osteoporosis (Studd 1990, Sands 1997, Worboys 2000). There are multiple studies showing the long term reduction in breast cancer in women using pellet therapy (Notelovitz 2004, Glaser 2013) rather than the increase in the incidence of breast cancer that has been associated with oral, synthetic methyl-testosterone (Tamimi 2006). Even after over 20 years of therapy with hormone implants, the risk of breast cancer is not increased (Gambrell 2006).

Hormone replacement therapy by pellet implantation has been used with great success in the United States, Europe and Australia since 1938, and has been found to be superior to other methods of hormone delivery (Greenblatt 1949, Mishnell 1941, Stanczyk 1988). It is not experimental. Pellets deliver consistent physiologic levels of hormones and avoid the fluctuations of hormone levels seen with other methods of delivery like pills, creams, gels and synthetic injections (Greenblatt 1949, Thom 1981, Stanczyk 1988). Pellets are superior to oral and topical hormone therapy with respect to relief of menopausal symptoms (Staland 1978, Cardoza 1984).

Pellet HRT and Cardiovascular Improvement

Hormones delivered by the subcutaneous implants bypass the liver, do not affect clotting factors and do not increase the risk of thrombosis (Notelovitz 1987).
Testosterone and estradiol delivered by pellet implantation, does not adversely affect blood pressure, glucose or liver functions (Burger 1984, Barlow 1986, Notelovitz 1984, Stanczyk 1988, Davis 1995, Sands 1997, Seed 2000, Cravioto 2001). In fact, testosterone and estradiol improved lipid profiles by reducing cholesterol, reducing triglycerides, and increasing HDL cholesterol (Davis 2005). This has positive benefits on the cardiovascular system.

Benefits of HRT with Implants

Hormone replacement therapy (HRT)with estradiol and testosterone implants is superior to oral and topical (both the patch and gel) hormone replacement therapy for bone density (Savvas 1988, 1992, Davis 1995, Anderson 1997).

Do HRT pellets prevent bone loss?
The pellets not only prevent bone loss but also actually increase bone density (Savvas 1988, Studd 1990, Garnett 1991, Savvas 1992, Naessen 1993, Holland 1994, Studd 1994, Davis 1995, and Anderson 1997).

Replacement Therapy for Low Testosterone
Is pellet therapy effective Testosterone replacement therapy in men with subcutaneous implants (pellets) has been shown to be extremely effective, convenient and safe (Handelsman 1990, 1992, 1997, Kelleher 2001, 2004, Conway 1988,Jockenhoval 1996, Zacharin 2003, Schubert 2003, Dunning 2004). The continuation rate continues to be 93% or above. This is excellent for long term compliance and exceeds the continuation seen with all other treatments for andropause.

Hormone Dosage in Pellet Therapy

The routine doses of testosterone delivered by pellet implantation in recent studies are between 1000 and 2400 mg in men. The pharmacokinetics and pharmacodynamics are well established showing that these doses deliver reproducible physiologic levels of testosterone for 4-6 months. A 6-9 mg daily production of testosterone is a ‘physiologic’ level produced by the testicle. Peak serum testosterone levels with the implants are usually seen at month one.

Expected Improvement in Testosterone Levels after Month 1 Pellet Therapy

Therapeutic testosterone levels at month one, are expected at the upper limits of normal for healthy young males (900-1100 ng/dL). These levels are necessary to protect the brain from Alzheimer’s disease, diabetes, heart disease, prostate cancer, osteoporosis and all-cause mortality (Zitzman M. J Clin Endocrinology 2006). By month 4 to 5 testosterone levels drop to below 500-600 ng/dL at which time symptoms return and the pellets are reinserted. Each individual has their own reproducible levels where symptoms return.

Testosterone implants have been used in women in 5 continents for decades. Doses used in studies are as low as 50 mg and up to 225 mg Glaser and Dimitrakakis Maturitas: 2004). Normal testosterone levels are not established in females (Fertility and Sterility 2002). Symptoms return when testosterone levels reach the upper end of endogenous ranges (Burger 1985). End organ response to testosterone remains optimal (i.e., relief of depression, increase in bone density, relief from insomnia, relief from aches and pains, lessened anxiety, improved memory and concentration, increased energy, etc.) when testosterone levels at 4-6 weeks after pellet insertion are 150-250 ng/dL. Steady state is subsequently achieved at approximately half of these levels equaling 80-120 ng/dL, which is in the physiologic to slightly supra-physiologic range. It is of primary importance to titrate the dose to achieve symptom relief and minimize side effects, not to achieve some phantom blood level. As women age, testosterone receptors become less responsive and more often than not, higher levels of testosterone are required to achieve the clinical outcome desired of symptom relief and long-term protection to the brain, breast, heart and bones. Some women require upwards of 300 ng/dl to achieve these results. Side effects from testosterone therapy in women are more of a nuisance and are reversible; there are no known long-term adverse effects in women, even at supra-physiologic levels.

Patient compliance becomes a non-issue using the pellet modality.

How the Pellet Preparation Process Impacts Hormone Therapy Results

The method of sub-cutaneous hormone replacement therapy has been consistent throughout the literature. What was needed was a refinement of the pellets themselves. BioTE® Medical has established the “gold standard” in pellet preparation. We standardized the process and then used independent labs to assure proper density, purity, potency, sterility, dissolution rate, solubility and temperature tolerance; all of which significantly affect how well a patient responds to the therapy. This extensive safeguard allows us to supply pellets with only 3% tolerance for potency (i.e. our pellets when prescribed will nearly match that requested). This is in contrast to prescribed pills and creams which may have 10-30% tolerance. We use no fillers in our compounded pellets and as such purity testing is superior. No pellets are dispensed until sterility is certified and assured.
